DR 8404 (or DR 8403 for 3.2% retail)
DR 8404-1

Diagram of store

Lease agreement

Purchase agreement
Affidavit Concerning Criminal History

Colorado State application and license fee
Town of Foxfield application and license fee

Check for $38.50 made out to Colorado Bureau of Investigation for background check

Submit two copies of application, including the items listed above, to the Town Clerk:

DR8442 Report of Changes Form (2 copies)
DR8404-1 Individual History Record (2 copies)
Affidavit Concerning Criminal History
Colorado State application fee
Town of Foxfield application fee
Check for $38.50 made out to Colorado Bureau of Investigation for background check
